NCHS Leading Causes of Death by State, 1999-2017
NCHS "Leading Causes of Death: United States" extract from data.cdc.gov (dataset bi63-dtpu): deaths and age-adjusted death rates per 100,000 (2000 US standard population) for the 10 leading causes of death plus all causes, for the United States, the 50 states, and the District of Columbia, 1999-2017. It covers only those 11 cause groups, with no age, sex, or race detail, and ends in 2017; it is not the full CDC WONDER mortality database. NCHS stopped updating this file after 2017; for 2014-2024 state deaths and age-adjusted rates by cause (17 causes, the same death counts on the overlapping years), use cdc/state-mortality-by-cause. Maternal causes are not included. For maternal deaths by state and year from WONDER, use cdc/wonder-maternal-deaths-by-state.

Primary Care Health Professional Shortage Areas (HPSAs)
Health Professional Shortage Area (HPSA) designations for primary care from HRSA. Contains shortage area scores (0-25, higher = more shortage), designation types, provider ratios, and population data with county FIPS codes and geographic coordinates. Primary care HPSAs are used to determine eligibility for National Health Service Corps placements and Medicare incentive payments. Each row is one component of a designation (a county, county subdivision, census tract or facility), not one shortage area: a single HPSA can span dozens of rows, and HPSA-level fields (score, designation population, FTE, shortage) repeat on every one of them. The file also keeps every withdrawn designation back to the 1990s, which is most of the rows. To count shortage areas in effect today, keep hpsa_status in ('Designated', 'Proposed For Withdrawal') (a designation proposed for withdrawal stays in force until HRSA withdraws it) and count distinct hpsa_id; the active-hpsas view does exactly that, one row per HPSA, and is the default. Summing population over raw rows double counts.
