Transparency in Coverage - Claims Denials & Appeals

When an insurance company denies your claim, you have the right to appeal. This dataset shows how often that actually happens across different insurers and plans. You can see the denial rate (what percentage of claims get rejected), the most common reasons insurers use to deny claims, and crucially, what percentage of appeals actually succeed. If an insurer denies most of your neighbors' claims and overturns very few on appeal, that's a sign the company is harder to work with. The data breaks down by plan type, coverage level, and year, so you can compare insurers in your area or track whether denial rates have gotten better or worse over time.
