National Ambulatory Medical Care Survey (NAMCS)

Real patient visits to community health centers, tracked by the CDC. Each record is one person's appointment and includes their age, race, sex, and what health problems the doctor diagnosed that day. The CDC samples thousands of visits every year from health centers across the country, so you can see patterns in who comes in, what's wrong with them, and what the doctor finds. This is actual electronic health record data, not surveys or estimates, which makes it pretty reliable for understanding what kinds of health issues people are dealing with at these clinics.
